It's mainly because of how I'm built and grew. My doctor says that I am really athletically built, and as a result, as I was growing, my arm bone was moving around too much in my rotator cuff. The group of four (or maybe five) muscles around it were stretched out too much in an attempt to prevent my arm from falling out. They are always stretching now. They are a lot stronger than most people's because of the stress, but they are also being torn up because they never get a chance to rest. It's called glenohumeral instability (gh instability) and I have it worse than most people because it is all of my muscles there. As a result of this, I also have scapular dysfunction, which basically means that my shoulder blades are rotated a bit and, in my case, are two inches lower than they should be. I've always felt tenseness in my shoulders, but a few years ago they started hurting daily, especially during my sports. So it's pretty much something I've been dealing with for all of my life.
