I am playing game went afk for 5 min or so come back and I have message that my hospital is full, but I have no report of an attack

your troops either died from an attack you sent or they starved

or it was a late  hospital full message...

It could be because of those Shadow Mercenaries... you hire them for rubies and if you send them off no one will know whom set the attack, but I dont think it explains why you didn't see the report? Must have been some sort of glitch or the Mail man lost the Letter.
